Workforce Well-Being Survey

The Office of Well-Being has created a comprehensive survey examining key domains of workforce well-being using a variety of evidence-based, actionable questions for use within the University of Michigan Health System. This tool differentiates from high-level engagement surveys by probing deeper, enabling leaders to make more informed decisions by targeting specific root causes impacting well-being. The survey contains questions spanning various domains of well-being. The estimated time for completion is less than 10 minutes.  For a PDF version of the survey, click HERE.

An electronic version of the survey is also available in Qualtrics. Please contact the Office of Well-Being if you would like a pre-made survey file in Qualtrics to be provided (Office-of-Well-Being@med.umich.edu).

6-Item Well-Being Pulse Survey

This short  survey is created for anyone employed at the University of Michigan Health system. It features 6 key questions for a brief, actionable report on workforce well-being. The estimated time for completion is roughly 1 minute.  For a PDF version of the survey, click HERE.
